About Us
The CSN serves primarily individuals experiencing significant mental health challenges and also those with co-occurring substance use disorders. Most of the individuals served by the CSN lack financial resources and may face a variety of stressors associated with poverty, including inadequate housing, medical care, social support and employment.

Interagency Referrals
To obtain a hard copy of the Inter-Agency referral Form, please call 203-974-7082.
Inter-Agency Referral Form PDF Note:
The Inter-agency Referral From must be submitted by the client's clinical provider. The referring clinician and client will then be invited to attend a meeting with CSN providers to discuss the client's goals. If appropriate, a recommendation for services will be given and the client will be invited to visit the program to learn more.
