YM CARE Center Kicks Off a New Era in Patient Access

On Jan. 29, Yale Medicine began an exciting new chapter in its efforts to provide patients with simple, supportive, and coordinated access to its world-renowned physicians.

On that date, the Yale Medicine Coordination, Appointment, Referral & Engagement (CARE) Center was launched in three clinical areas: Nephrology, the Diabetes Center, and the Bone Center.

The goal of the YM CARE Center is to make it easier for patients and referring providers to access Yale Medicine services. Through a single point of entry, patients and referring providers will have access to specialty-specific staff who will assist them with such services as ambulatory appointment scheduling, referral management, care coordination, nurse triage, and transition of care planning.

Following the Jan. 29 launch of Nephrology, the Diabetes Center, and the Bone Center, additional Phase I departments and sections—including Orthopaedics, Pediatrics and Pediatric Surgical Services, Urology, Neurology, and Ophthalmology—will be transitioned to the YM CARE Center at a cadence of approximately every two-to-four weeks from now through the summer. Two additional phases of the project will follow, integrating the entire practice into the CARE Center over the next two years.

The YM CARE Center is located in newly renovated space at 25 Science Park. Expanded hours of operation from 7 a.m. to 7 p.m. Monday through Friday will serve to better address the needs of patients and families.

As employees gradually transition to 25 Science Park, they will find bright, contemporary work spaces with plenty of amenities, including desks that can be automatically raised and lowered to go from sitting to standing positions. The facility includes a large room for training, meetings, and social gatherings; a “Quiet Room” for relaxation; a “grab and go” food concession; an attractive dining space; and a fully equipped gym that is available to employees free of charge.
