Clinical Mission

  • 00:04Good morning.
  • 00:05So doctor McGovern sends her apologies.
  • 00:08She she last night came down with
  • 00:09something and she said if I could
  • 00:11pinch and she doesn't want to be
  • 00:12here spreading germs of course.
  • 00:13But so I will,
  • 00:14I will do the presentation on
  • 00:16the on the clinical mission and
  • 00:18really on Yale Medicine which
  • 00:20is the the clinical practice of
  • 00:22the of the School of Medicine.
  • 00:25It comprises of the of the 19 clinical
  • 00:28departments and and the ALE Cancer Center,
  • 00:31it's actually now more than 1800
  • 00:33faculty with the this new cohort here.
  • 00:35So we we crossed that barrier this
  • 00:39year 840 advanced practice providers,
  • 00:40more than 1900 staff and we are
  • 00:43through 100 locations throughout the
  • 00:45state with 300 practice sites and
  • 00:48that encompasses about 2.6 million
  • 00:50encounters annually with a budget of
  • 00:52about one point 1.5 at 1.4 billion.
  • 00:54And I have all that that the chairs
  • 00:56listed there I'm sure they're
  • 00:57part of your department.
  • 00:59These are the clinical
  • 01:00chairs School of Medicine.
  • 01:03So, so young medicine which
  • 01:05is the fact to practice plan.
  • 01:07We have our mission and vision here
  • 01:09which is really to deliver exceptional
  • 01:11medical care in a nurturing environment
  • 01:13to each of our patients every day.
  • 01:15And our vision is to advance medicine
  • 01:16for the good of our patients,
  • 01:18colleagues and community.
  • 01:19And we do this with this circle
  • 01:22which are our five pillars really
  • 01:24to be inquisitive and really
  • 01:25learn from our patients,
  • 01:26be innovative by using that and
  • 01:30translating into research from the
  • 01:32bench to the to the bedside and and
  • 01:35in factoring the the patient as a
  • 01:36whole rather than their diseases.
  • 01:38So really the human factor and while
  • 01:40we're doing this work collaborating
  • 01:41both internally at at the School of
  • 01:43Medicine but also with our health
  • 01:45system partners and doing it in a
  • 01:47in a passionate way and and really
  • 01:49in the middle are caregivers,
  • 01:51Folks that are in this room are our
  • 01:53patients And then by working together we
  • 01:54can really achieve the best care possible.
  • 01:59The Yale Medicine Board itself
  • 02:02is led by by the department
  • 02:04chairs and four atlarge members.
  • 02:07Each year there is one chair that's
  • 02:10chosen to to chair the board and
  • 02:12and right now it's Doctor Nita
  • 02:14Huja who's the chair of surgery.
  • 02:16Proposals for Yale Medicine are developed
  • 02:18in multiple different committees
  • 02:20which are led by and appointed diet.
  • 02:21So for example,
  • 02:22our Finance Committee is led by Doctor
  • 02:24Glaser who's the Chair of Radiation
  • 02:26Oncology along with Ian Burchell
  • 02:28who's our Chief Financial Officer.
  • 02:30Our Network Strategy Committee is led by
  • 02:32Doctor Liu who's the Chair Pathology,
  • 02:33along with Yolanda London who's
  • 02:35our Chief Strategy Officer.
  • 02:36Then we have a few other committees there
  • 02:38as well and that is how we function.
  • 02:40So everything we do within
  • 02:41Yale Medicine is reviewed,
  • 02:42approved by the board which is
  • 02:44which are the chairs as well As
  • 02:45for at large faculty members.
  • 02:50So these are some of the services
  • 02:51supported by Yale Medicine.
  • 02:52So those that'll be will
  • 02:54be touching patients.
  • 02:54If you have clinical duties,
  • 02:56we will be supporting you along
  • 02:58the way including revenue cycle
  • 03:00and the billing component.
  • 03:02Pay your contracting quality of
  • 03:03safety under doctor Deb Rhodes
  • 03:05who's the Chief Quality Officer
  • 03:07not just for Yale Medicine but
  • 03:09for also Northeast Medical Group
  • 03:10which is Doctor Brown mentioned
  • 03:12that group on the hospital side
  • 03:13as well as the health system.
  • 03:15We do see oversee ambulatory
  • 03:17operations and we'll support you in
  • 03:18your clinics and also try to optimize
  • 03:20those those operations through
  • 03:21our clinical optimization team.
  • 03:23We're involved in practice acquisitions
  • 03:25which is really working with the
  • 03:26chairs and section chiefs on any
  • 03:28opportunities throughout the state.
  • 03:29IT support and services,
  • 03:31registration of patients,
  • 03:33any compliance issues that may
  • 03:35come up both internally related to
  • 03:37billing or clinically and of course
  • 03:39risk management and malpractice.
  • 03:41We do provide support for access
  • 03:42and I'll give an example of
  • 03:44the care center in in a minute.
  • 03:46And then we have a population
  • 03:47health team looking at how we can
  • 03:49care for patients not just within
  • 03:50within New Haven regionally,
  • 03:51but also across the state and partnering
  • 03:53with the health system on those initiatives.
  • 03:55And there there's a large
  • 03:57component of strategic growth.
  • 03:58I trained here myself and
  • 04:00when I was a resident,
  • 04:01it was very New Haven focused.
  • 04:03And now of course,
  • 04:04we're across the entire state and there
  • 04:05are plans to possibly move N to the state
  • 04:07in Waterbury and a few other hospital system.
  • 04:09So there's always growth in
  • 04:11a system like this.
  • 04:13And then of course,
  • 04:14the marketing piece,
  • 04:14we want to make sure we market
  • 04:16the excellent care we provide,
  • 04:17the research as well as the
  • 04:19educational initiatives that we have.
  • 04:21So I'll I'll use the care center
  • 04:23as an example.
  • 04:24Some of you will,
  • 04:25if you have outpatient clinics,
  • 04:26you will interact with the care
  • 04:28center if your section or clinic
  • 04:29is part of the care center.
  • 04:30So the care center itself is,
  • 04:32it's an acronym that stands for Coordination,
  • 04:34Appointment,
  • 04:35Referrals and Engagement Center.
  • 04:37It was set up back in 2019 and
  • 04:40now has integrated 22 specialties.
  • 04:44They've answered more than I think
  • 04:46it's actually close to 4 million calls,
  • 04:47makes more than 3 million appointments.
  • 04:50They do manage more than 1800 templates
  • 04:52and I process more than 10,000
  • 04:54new patient referrals and and done
  • 04:56more than 9000 transitions of care.
  • 04:58So it's a really,
  • 04:59really large operation and if
  • 05:01you have outpatient clinics,
  • 05:02it's likely that you will
  • 05:03interact with the care center.
  • 05:04This is a function that Yale
  • 05:06Medicine provides to to the
  • 05:08faculty and to the departments.
  • 05:10Another function will actually be a
  • 05:12monthly dashboard that you will receive.
  • 05:14If you don't let me know,
  • 05:15just shoot me an e-mail,
  • 05:16we'll make sure you get it.
  • 05:16But this is a monthly dashboard from
  • 05:18Yale Medicine that gives you data.
  • 05:20This is just two sample pages
  • 05:22from there related to
  • 05:23your productivity RVU's,
  • 05:25how many patients you're seeing,
  • 05:26charges, collections,
  • 05:27that's sort of the front page.
  • 05:29And then there's information
  • 05:30about your clinics.
  • 05:31If you do have outpatient duties,
  • 05:32you'll be able to see how long
  • 05:34is it taking patients to see you,
  • 05:35what percentage of visits are telemedicine.
  • 05:38So all those sort of data points
  • 05:40will be there on a monthly basis.
  • 05:41And this is for our physicians and our APP's.
  • 05:44And again,
  • 05:45if you if you don't receive this,
  • 05:47please let me know directly,
  • 05:48just e-mail me or e-mail your section
  • 05:50chief for chair and they'll take care of
  • 05:54it. And and Doctor Brown mentioned that
  • 05:56we really work in an ecosystem and and
  • 05:58this slide sort of gives you an idea of
  • 06:01what that health ecosystem looks like.
  • 06:02So you have the, the school and Yale
  • 06:05Medicine there in the middle and
  • 06:06really our key partner is the only
  • 06:08Heaven health system which is truly an
  • 06:10integrated system across the state.
  • 06:11So of course you have Yale New
  • 06:13Haven Hospital which is right here,
  • 06:14our largest academic affiliated
  • 06:16hospital with just under 1700 beds.
  • 06:18It's actually considered one of the
  • 06:20largest hospitals in the country
  • 06:21with the two campuses.
  • 06:23We have Lawrence and Memorial
  • 06:25Hospital in New London,
  • 06:27CT Westerly Hospital in Rhode Island.
  • 06:30So we do have a flag in in Rhode Island,
  • 06:33Milford Hospital,
  • 06:34Bridgeport Hospital and then and
  • 06:36then Greenwich Hospital and she
  • 06:38mentioned Northeast Medical Group,
  • 06:40any MG, so any MG is the employed
  • 06:43physician group of the health system.
  • 06:45It was, it was,
  • 06:46it was created in the late sort of 2000,
  • 06:492007,
  • 06:492008 and is primarily composed of primary
  • 06:53care majority of the clinicians and
  • 06:55APP's that are in any MG or primary care.
  • 06:58But they do have specialists as well.
  • 07:00And this year for the first time
  • 07:02we've actually come together under
  • 07:04the aligned clinician enterprise.
  • 07:06I know her slide had the physician
  • 07:08enterprise always changes to
  • 07:10clinician to to recognize our
  • 07:12APP's and our other colleagues.
  • 07:12So it's called ACE.
  • 07:13So we're all aces in the books of of these,
  • 07:15of these groups.
  • 07:17So they're going to come together
  • 07:19really closely and and and it makes
  • 07:21sense over time really the primary
  • 07:23care component and specialty component
  • 07:25really need to sort of function
  • 07:26together and this will allow us to
  • 07:28align much closer with our health
  • 07:30system partners and Doctor McGovern is
  • 07:32going to lead those groups together.
  • 07:34We have of course other key
  • 07:35partners as well in the state.
  • 07:36So the VA healthcare system,
  • 07:39many of you may go there.
  • 07:40I actually started there
  • 07:42myself in West Haven.
  • 07:43It's a great system and we have our trainees,
  • 07:46our students, fellows,
  • 07:47residents also go there.
  • 07:48So it's a really,
  • 07:49really close partner for us.
  • 07:50Check in mental Health Center,
  • 07:52which is right here in New Haven,
  • 07:53it's another partner.
  • 07:54And then we do have key relationships
  • 07:56with other hospitals that are not
  • 07:57in the only heaven health system
  • 07:59but are throughout the state,
  • 08:00Griffin Hospital and a few others
  • 08:02that we also work with very closely.
  • 08:05So I won't go into this too much.
  • 08:08This is the old, I can say it's
  • 08:09the old strategic framework now
  • 08:11because the one doctor brown chair
  • 08:13just came out about a month ago.
  • 08:15But we've had a strategic framework
  • 08:17with the health system for for many
  • 08:19years and it's really focused around
  • 08:22these pillars in terms of providing
  • 08:24care and inspired culture of excellence,
  • 08:26ensure consistent care signature you
  • 08:27you you'll still hear the care signature
  • 08:30piece a lot from the health system and
  • 08:32and Yale Medicine enhanced clinical
  • 08:34services manage the cost of care.
  • 08:36This will be important of course
  • 08:38there's extreme pressures on all
  • 08:39healthcare systems right now and
  • 08:41as the cost continue could go up,
  • 08:42but our reimbursements continue to go down.
  • 08:45So.
  • 08:46So you'll hear about this both
  • 08:47from the health system,
  • 08:49the school and Yale medicine and really
  • 08:51sustaining a financial strength there.
  • 08:53There is actually initiative right now
  • 08:55that the health system has kicked off
  • 08:57cause called strengthening our core,
  • 08:59really related to improving our efficiency,
  • 09:01reducing our cost and really delivering
  • 09:03high quality, excellent care.
  • 09:06So you'll hear more about the
  • 09:07strategic framework.
  • 09:08I know Doctor Brown just mentioned it,
  • 09:09but there's going to be more communication
  • 09:11about this in the coming weeks and months.
  • 09:13And I would say in my last slide.
  • 09:16So this sort of puts all the
  • 09:17puzzle pieces together, right?
  • 09:18These are the, I think,
  • 09:20institution groups here really
  • 09:22to support you and our patients.
  • 09:25You have Yale Medicine, School of Medicine,
  • 09:27Yale University and the Only Heaven
  • 09:30Health System.
  • 09:31And as you embark on this journey,
  • 09:33we are here to support you and our patients.
