Child Study Center Grand Rounds: "Closing Health Equity Gaps in Healthcare: A Statewide Approach to Eliminating Racial Disparities"

Evidence directs healthcare professionals to engage with the community to address root causes of health and inequities—and to do so in partnership.

Organizations who understand the importance of addressing implicit bias, embracing diversity, and promoting health equity are more adequately equipped to optimize retention and quality community outreach to at risk populations. Addressing health equity requires intentionality and long-term commitment.

In this Grand Rounds, The Connecticut Network of Care Transformation grant (CONNECT) focused on integrating child serving systems will be discussed. Partnerships between families, state agencies and service providers will further be discussed with a specific focus on ways to engage with the Culturally and Linguistically Appropriate (CLAS) Workgroup.

On overview of how to move from discussions on eliminating social barriers to quality health care to develop actionable data driven strategic approaches to improving outcomes for those serving vulnerable populations will be the focus of this presentation.
