Overview

Autumn Church is a peer support specialist within the Yale ACCESS Mental Health team who utilizes her lived experience to provide coordination services for children, teens, families, and young adults with a wide range of emotional, behavioral, and social concerns. Through her role, she utilizes expertise across disciplines to support children, families, and young adults in accessing connections to community providers and educational opportunities.

Church provides thorough case management assessments tailored to families referred to ACCESS Mental Health through their pediatricians. In her work, she focuses on building trust and creating a safe, welcoming space where families can share their experiences and feel heard.

Church serves as a lecturer at Yale School of Medicine, where she participates in training programs focused on child and adolescent mental health.

Church is pursuing a graduate degree in social work at Southern Connecticut State University, where she completed her undergraduate degree in the same field. She was awarded the Henry Barnard Distinguishing Student Award for her excellence in academic and community support, as well as the Michelle Wheeler Social Work Service Award for her role in legislative advocacy through the NASW-CT Education and Legislative Action Network.
