Forms and Information to Submit

New Fellow Information Sheet

This information is collected in order to create your Yale University profile, set-up your 'yale.edu' email, and establish your HR/payroll record. Your fellowship coordinator should have already been in touch with you about this form. If you haven't already done so, please complete it and submit it to your fellowship coordinator (see contacts tab).

Yale University Background Checks

The University will be conducting a background check on all University-paid fellows before you begin your training. In order for us to start that process, we need a current email address. Your fellowship coordinator should have already been in touch with you about this matter. If you haven't already done so, please submit your email address to your fellowship coordinator (see contacts tab).

Once the background check process has been initiated, you will be contacted directly by the company, HireRight, Inc. When completing the background check form, for education, please list only your medical school.

Forms To Be Completed After You Arrive

You will receive hard copies of these forms during orientation and will be asked to fill them out during one of your orientation sessions. If you are able to complete these forms ahead of time and bring them to orientation, that would be very much appreciated (and will save time during orientation). The forms are below.

Controlled Substance

Register for State of Connecticut Controlled Substance Practitioner Registration

You will receive an email from the State of CT Department of Consumer Protection (DCP) after you have been added to the Yale New Haven Hospital residency training permit. This email will request that you apply for a State of Connecticut Controlled Substance Practitioner Registration. Please follow the link in the email and apply for this license.

Click "Apply Online!" and register for a username and password. (Please save this information as it will be needed again when you apply for a State of CT medical license during your PGY-2 year and for renewing your licenses.) Log out and log in back in (after verifying your email address). Select "Drug Control" and then select "Controlled Substance Registration (CSR) for Practitioner." "Start" application for CSR. Please use a personal email when creating the account (not your medical school email).

Here are some tips for completing the application:

  • Enter your name exactly as it appears on your Social Security Card or ECFMG Certificate.
  • Registration Classification: Resident/Intern

  • Question regarding Connecticut Prescription Monitoring and Reporting System (CPMRS): No (We will have you start registering for CPMRS in early July.)

Housing in the New Haven Area

Those looking for housing may be interested in this link to the Yale Off Campus Housing website. Even without a Yale Net ID, you can still log on by creating a profile, which is quick and easy to do. When asked what your position will be at Yale, just answer “other.” https://housing.yale.edu/

Health Benefits

If you are a University-paid fellow, you will be informed of the options for health benefits during orientation and directed on how to sign up for them online. If you have general questions about health care options that you would like to have answered before you arrive, please contact the Employee Service Center at 203.432.5552 or employee.services@yale.edu.

Hint: When asked what your job title is, answer “Other Academic.” This is your title for purposes of health benefits. If you still have questions you can always contact Emily Johnson.
