• Letter/email stating your desire for an appointment

• List of primary Section/service for teaching; please include other services you are willing to teach
• current academic curriculum vitae in the medical school format

The application is reviewed by the Voluntary Faculty Committee and Service/Section Chief, who in turn make a recommendation to the Chairman (or entire Appointments and Promotion Committee, for the level of Associate Clinical Professor or Clinical Professor).
