The Schwartz Rounds program offers opportunity for discussion in panel format topics with social and emotional dilemmas faced in patient care. The committee chooses relevant topics and panelists and moderates the discussion.

The committee developed a new care coordination and transitions of care template to enhance patient safety, establish clear expectations and educate residents on best practices for transitions of care.

The committee used resident feedback to design and pilot a new admitting structure across all inpatient admitting teams using an iterative process at VA Connecticut.

I coordinated and facilitated resident experience rotating at Fellowship Place. Activities included preparing and serving food, providing informal education to the clients on health topics, including but not limited to nutrition, exercise, and diabetes, and answering questions. We discuss barriers to access and care in the context of this experience.
