    Biography

    Jonathan is assistant professor of medicine and course director for the Prevention, Populations and Patient-Centered Care master course in the Yale School of Medicine Physician Associate Program. He has practiced internal medicine in the Yale New Haven Health and Yale University health systems for 28 years. He currently practices at the Yale Medicine Diabetes Center, the joint clinical practice of the Yale School of Medicine and Yale New Haven Health System. He works with medically complex patients on diabetes management and specializes on the integration of diabetes technologies and personalized lifestyle medicine.

    Previously, he practiced as a hospitalist, primary care, urgent care and athletic medicine clinician. Throughout his career, Jonathan has remained active in PA education, program administration and leadership. Chief academic interests and expertise include chronic disease management, AI care innovations, lifestyle medicine, comprehensive wellness, psychosocial communication, ethically vulnerable populations and health equity.

    He has remained committed to advocacy and professional service. As past chair of legislative affairs for the CT Academy of PAs, he has been responsible for leading major health care reform efforts resulting in modernization of the PA Practice Act and improved patient care for CT residents. He is the recipient of several awards for his clinical service, teaching and professional service including recognition as a distinguished fellow by the American Academy of Physician Associates.

    He previously served as Director of Didactic Education for the Yale Physician Associate Program. The major responsibilities of the Director of Didactic Education included the development, coordination and delivery of academic curriculum; maintenance of national accreditation standards; director of Pharmacology and Prevention, populations and patient-centered care and co-director of Clinical Medicine master courses; academic advisor and mentor to graduate medical students; and, participation in School of Medicine and PA Program committees: Academic Progress, Admissions, Educational Policy and Curriculum and Program Assessment.

    Research

    Overview

    My academic and scholarly interests focus on improving clinical outcomes and quality of life for patients with chronic diseases through the clinical practice guidelines development and the design, evaluation, and delivery of effective graduate medical curriculum.

    I am nationally known for leading diabetes care and technology workshops and presentations for the last 5 years. In 2025, I coauthored a novel clinical practice guideline, entitled “Lifestyle Interventions for Treatment and Remission of Type 2 Diabetes and Prediabetes in Adults.” with a select group of multi-disciplinary experts. This landmark guideline offers clear, actionable, and evidence-based strategies for helping individuals with prediabetes or type 2 diabetes optimize their lifestyle behaviors, sustain positive changes, and potentially reduce or eliminate the need for medications.

    I was also thrilled to have co-authored with first author and graduate PA student mentee, Jay Gandhi, a retrospective study on the use of telemedicine in diabetes care delivery published by the Journal of Diabetes Science and Technology. Finally, I had the privilege of collaborating with Dr. Anika Anam (PI) and Dr. Donna Windish, Program Director of the Yale GIM Medical Education Fellowship on a novel curricular initiative in the Yale PA Program. In late 2025, the study, "Implementing an Innovative Transgender and Gender Diverse Health Curriculum for Physician Assistant (PA) Students" was published in AACE Endocrinology and Diabetes.

    Additionally, I worked with a select group of providers to develop two YNHHS Care Pathways for pre-diabetes and type 2 diabetes and have collaborated with the American Academy of Physician Associates (AAPA), the Endocrine Society, the American Academy of Family Physicians (AAFP), and the American Association of Nurse Practitioners (AANP) to develop enduring CME curriculum on diabetes management. Locally, I have collaborated and published educational research abstracts and poster presentations with several Yale faculty for the Obesity Society, the American College of Lifestyle Medicine and for the Yale Medical Education conference on novel obesity, transgender care and lifestyle medicine curriculum respectively.

    Clinical Care

    Overview

    Jonathan Weber helps patients with diabetes at Yale Medicine’s Diabetes Center manage all aspects of their disease. He specializes in treating patients with kidney issues or other complications caused by diabetes.

    “Working together with patients to find the best solutions to help them manage their diabetes and prevent diabetes-related complications is very rewarding,” Weber says.

    He says he enjoys helping patients take steps to gain more control over their diabetes and make lifestyle changes such as eating healthier or exercising more to improve their overall health.

    “I am interested in sharing what we know about lifestyle medicine along with current and emerging prescription therapies that meet patients’ needs and fit best into their lives,” he says. “The burdens that come with any chronic disease can be challenging. With a consistent and strong ‘therapeutic alliance’ between the patient and provider, I believe we can overcome any challenge together.”

    Weber has practiced internal medicine for more than two decades in the Yale New Haven Health system.
