Hospice & Palliative Medicine Fellowship

To prepare the next generation of hospice and palliative medicine clinicians to serve the palliative care needs of patients, families, and practicing clinicians across diverse communities and settings through rigorous academic clinical training, a transdisciplinary team care model, a focus on clinician wellness, and a commitment to high value, equity focused care.

Aims:

1. To train fellows to provide excellent clinical palliative care to all relevant patient populations across clinical settings while acting to address health inequities in care systems
2. To mentor fellows toward specific expertise in patient/family-centered care, cultural humility and inclusion, advanced symptom management, serious illness communication skills, transitions across palliative care settings, and wellness/self-care
3. To prepare fellows to practice and lead within transdisciplinary palliative care teams, leveraging the expertise of Yale's interprofessional community to optimize coordinated palliative care delivery and team care
4. To provide fellows with a robust, academic setting to promote models of lifelong palliative care learning, teaching, advocacy, and leadership

• Yale Palliative Care Program founded in 2011; Fellowship launched in 2014
• Fellowship format: 1-year, ACGME-accredited clinical fellowship in the Department of Internal Medicine
• Positions: 3 palliative care track; 1 combined palliative care–geriatrics track
• Interdisciplinary team training: physicians, Advanced Practice Providers, nurses, social workers, pharmacists, psychologists, and bereavement counselors in both inpatient and outpatient settings
