Download the directions to the Yale Children's Diabetes Program Clinic, which is located on the 2nd floor in the Pediatric Specialty Center.

Please let us know where you are moving and we will be happy to help you find a new provider. If you already have a new office in mind please let us know at your last clinic appointment so we can have you sign a records release form. We will then send your records along with a summary of your care to your new provider.
