New Haven Primary Care Consortium

The NHPCC offers primary care for children and adults. In collaboration with Yale New Haven Health and Fair Haven Community Health Care, Cornell Scott Hill Health Center (150 Sargent Drive, New Haven, CT 06510) oversees the adult medicine program that offer primary care, women’s care, and addiction treatment services for adults ages 19 and over. The Addiction Recovery Clinic and Transitions Clinic offer treatment services, including buprenorphine treatment and brief counseling, and are embedded in the primary care office.
