Hospice & Palliative Medicine Fellowship

Since 2011, the Yale Palliative Care Program has grown quickly to provide increasing clinical care and educational program support to the Yale New Haven Hospital and Yale School of Medicine community. We aim to provide high-quality, whole person care to our patients with serious illness and their families. The Yale Hospice and Palliative Medicine Fellowship is a 1-year ACGME-accredited clinical fellowship within the Department of Internal Medicine. We were excited to launch this fellowship in 2014-15. Our goal is to train diverse future leaders in palliative care education and practice in our robust palliative care learning laboratory.

During this intensive one year clinical training program, fellows develop expertise in symptom assessment and management, comprehensive care plans that honor patient preferences, coping and unique characteristics, as well as, advanced serious illness communication skills for conversations with patients, families, and other health care providers. Fellows are active members of our interdisciplinary team from day one, rotate at multiple community sites, and are coached in resilience practice. Mentorship is provided by a faculty of physicians who are board certified in Hospice and Palliative Medicine along with a full interdisciplinary team of palliative care experts.

The Yale Palliative Care Program also values educational innovation. Thus, we are delighted to support two combined pilot fellowship programs. Fellows completing these programs are board-eligible in both of the combined subspecialties.
