The physicians at Yale are using a state-of-the-art electronic medical record (EMR) that allows patients to access their personal health information through a product called MyChart. Patients using MyChart have access to information including appointment information, immunizations, lab results, medications and more, all conveniently accessed online over a highly secure connection designed to protect personal health information.

Creating a volunteer profile allows you to express interest in study categories, which helps our recruitment team match you to trials for which you may be eligible. It indicates that you are interested in volunteering for a study but it is not a commitment to do so. If you are already enrolled in MyChart:
To create a volunteer profile, log in to MyChart, then click on the link to create a volunteer profile.
If you are a patient at a Yale New Haven Health System site, but are not enrolled in MyChart:
Go to the MyChart Registration page and complete the form to request an access code. Once you fill out and submit the Activation Code Request, you will receive your access code within one business day.
If you are not a patient within the Yale New Haven Health System:
You will need to create a research profile in MyChart. Go to the MyChart registration page. You will need to request an access code. Once you fill out and submit the Activation Code Request you should receive your access code by email within 5 business days. The wait time is to verify your identity and protect your identity and privacy.
