Yale Plastic and Reconstructive Surgery Residents' Perspective

  • 00:00Welcome to young plastic and
  • 00:04reconstructive surgery. First off,
  • 00:06thank you for your interest in our program,
  • 00:08and congratulations on making it this far.
  • 00:09We thought we would try to answer a
  • 00:11few of your questions in this video.
  • 00:14We'll start off with where do we work?
  • 00:18Yale New Haven Health has many
  • 00:21locations throughout Connecticut,
  • 00:22but its main hospital is the
  • 00:24Yale New Haven Hospital,
  • 00:26which has 1541 beds.
  • 00:28Anna split between two campuses,
  • 00:30York Street and St Rays.
  • 00:33This is the main entrance of the York
  • 00:36Street campus, which also includes.
  • 00:38The Smile Cancer Center on the
  • 00:40North wing of the building.
  • 00:42And the Children's Hospital on the West side.
  • 00:46And then just a few blocks down
  • 00:49the road as SRC or Saint Rays,
  • 00:51which is an 18 minute walk away,
  • 00:53or a short shuttle bus ride away.
  • 00:56This hospital has more of a Community
  • 00:59Hospital feel and also includes.
  • 01:01The relatively new Mcgivney Center
  • 01:03where we perform a significant portion
  • 01:05of our outpatient hand surgery.
  • 01:09In our junior years,
  • 01:11we also spend time on general surgery
  • 01:14and burn at Bridgeport Hospital.
  • 01:16Which is about 30 minutes away.
  • 01:18The farthest you would have to drive
  • 01:20for work in this residency program.
  • 01:23The final hospital recover
  • 01:24is the West Haven VA.
  • 01:27Which is only about 10 minutes
  • 01:28from the main York Street campus.
  • 01:32So now that you know where we work,
  • 01:35you must be wondering who we work with.
  • 01:38We have 10 full time faculty covering
  • 01:41general reconstructive surgery microsurgery
  • 01:43including breast lower extremity and hand,
  • 01:47upper extremity craniofacial surgery
  • 01:49including orthognathic surgery,
  • 01:50Melanoma and skin cancer and
  • 01:53cosmetic surgery including through
  • 01:55the resident cosmetic clinic.
  • 01:58Doctor Persing is our section Chief,
  • 02:00Doctor Shazar program Director,
  • 02:02and doctor.
  • 02:03Perez itches are associate program director.
  • 02:05We also have the opportunity to work
  • 02:08with a great number of private practice
  • 02:11surgeons who are especially valuable
  • 02:14for esthetic surgery education.
  • 02:17We're also fortunate to be able to work with
  • 02:20our fellows in hand breast microsurgery
  • 02:22and craniofacial surgery next year.
  • 02:24We will actually be gaining a second
  • 02:27hand fellow as we combine forces
  • 02:30with our orthopedic colleagues.
  • 02:32We also work very closely with
  • 02:34medical students not only as
  • 02:35clerkship students and subies,
  • 02:37but also as research fellows.
  • 02:39Many Yale medical students take
  • 02:41advantage of the opportunity to
  • 02:42have a fully funded research here,
  • 02:44so we are lucky enough to have students
  • 02:47working with us full time every year,
  • 02:50both from Yale, Ain,
  • 02:52other institutions.
  • 02:53But we couldn't do our jobs without
  • 02:55our great aips who help us out both
  • 02:58on the inpatient and outpatient side.
  • 03:00They are best friends as interns
  • 03:02teaching us everything we need to know
  • 03:04and keep us afloat as senior residence.
  • 03:06Trying to run the service.
  • 03:10OK, so now that we've been talking
  • 03:12about work, what our schedules like.
  • 03:15As you can see here,
  • 03:17we spend a significant portion of our time,
  • 03:20even as junior residents on
  • 03:21the plastic surgery service,
  • 03:23we do have to spend some time in
  • 03:25our first few years learning from
  • 03:27our general surgery colleagues
  • 03:28as well as from other services
  • 03:31combined here under miscellaneous.
  • 03:33This includes neurosurgery, ents,
  • 03:34anesthesia and orthopedics in their first
  • 03:37year Mohs surgery in the second year.
  • 03:39Oh MFS.
  • 03:39In the third year an ocular
  • 03:42plastics in the 4th year.
  • 03:44And then in our final two years,
  • 03:46we have the unique opportunity to have
  • 03:48three months of elective time in each year.
  • 03:51Electives in the 5th year can be away.
  • 03:54Electives similar to sub internships.
  • 03:56While electrodes in the six year
  • 03:57must be at home and may include
  • 04:00aesthetic electives or head and
  • 04:02neck microsurgery electives.
  • 04:03This elective time can also be used
  • 04:06for research or even developing
  • 04:08curricula for medical students.
  • 04:12We have not been able to travel
  • 04:14this year as a result of Cove in,
  • 04:16but here is a list of all the
  • 04:18different away electives are
  • 04:19residents have pursued in the past,
  • 04:20ranging from those in Paris and Taiwan
  • 04:22to closer to home in New York City.
  • 04:27Let's focus a little bit on
  • 04:28our main service for a minute.
  • 04:30We typically meet around 6:00 AM for rounds.
  • 04:32Depending on our patient list and whether
  • 04:34or not we have morning conference,
  • 04:36in which case we may meet earlier.
  • 04:40After rounds we touch base with
  • 04:42the attendings, formulate plans,
  • 04:43write notes and pre op.
  • 04:45The first patients for the day.
  • 04:47The first case of the days,
  • 04:49generally around 7:15 AM.
  • 04:50If you are not assigned to cases,
  • 04:53you were there, help out on the
  • 04:55floor or head over to the clinic.
  • 04:57Every resident on service does a
  • 04:59minimum of 1/2 day of clinic per week.
  • 05:02Finally, the on call resident takes
  • 05:04over the console pager at 5:00 PM,
  • 05:06which is when everyone needs to run the list.
  • 05:08After we have discussed all
  • 05:10the patience of the day,
  • 05:12the chief typically then goes
  • 05:13at flat rounds with the on call
  • 05:15resident so everyone is on the same
  • 05:17page before the day team leaves.
  • 05:22A couple of important service details.
  • 05:26We have what we call panel every other week.
  • 05:29This includes plastic surgery,
  • 05:31call and hand call with
  • 05:33or without facial trauma,
  • 05:35call or hand call is shared with
  • 05:38orthopedic surgery while facial trauma
  • 05:40call is shared with Ian Tiano manifest.
  • 05:44During the day, our Konsult resident
  • 05:47is typically the second year resident.
  • 05:50Overnight we have an in house intern
  • 05:53call pool that we share with E&T.
  • 05:56As well as the night float
  • 05:58system for consoles.
  • 05:59This is typically staffed by pgy threes
  • 06:02and pgy force on the York Street service.
  • 06:06They are night float for one week at a time,
  • 06:09typically Monday evening to Monday morning.
  • 06:11The next week,
  • 06:12essentially working a 24 hour and
  • 06:14the first Monday then switching to
  • 06:16nights for the remainder of the week.
  • 06:18Their back to normal days in
  • 06:20the following Tuesday.
  • 06:23This is a home call system,
  • 06:26but residents are expected to
  • 06:28respond in a timely manner.
  • 06:30These residents cover the York Street
  • 06:32campus SRC an the VA at night.
  • 06:34An important note this night float
  • 06:36service was created just two years ago
  • 06:39at the initiative of our Chief residents.
  • 06:41This is reflective of the power of
  • 06:43the faculty, gives us to shape our
  • 06:46program into what we think is best.
  • 06:49We mentioned our conference is
  • 06:51a little bit earlier.
  • 06:52Here are more details.
  • 06:54We typically have three days with
  • 06:56some kind of conference.
  • 06:58On Mondays from 6:45 to 7:30.
  • 07:00We typically have console or
  • 07:02view upcoming cases indication,
  • 07:04conference or radiology rounds.
  • 07:06On Wednesday evenings from five to six,
  • 07:09we typically have grand rounds with
  • 07:11guest speakers or visiting professors,
  • 07:13or once a month we have quality
  • 07:16assurance or Eminem.
  • 07:17And then on Friday mornings we have
  • 07:19a dedicated 2 hours of didactic time.
  • 07:22Usually these are resident lectures
  • 07:24based on Aspet said net curriculum
  • 07:26or once a month we have a joint
  • 07:29craniofacial quality assurance
  • 07:30with TNT and OMA Fest.
  • 07:34We mentioned are visiting
  • 07:35professors a little bit.
  • 07:37This is a list of all the visiting
  • 07:39professors we've had in recent years.
  • 07:41These visiting professors are
  • 07:44amazing opportunities to learn
  • 07:46from greats in our field.
  • 07:48We typically have them spend 2 hours
  • 07:50of dedicated time with the residents
  • 07:52only after which they give a grand
  • 07:54rounds to the entire division.
  • 08:00And then after that it's party time we
  • 08:02typically take are visiting professors
  • 08:04at to dinner and drinks it more.
  • 08:06Ease of private club at Yale since the
  • 08:081800s were the tradition is to share large
  • 08:11goblets of mysteriously colored drinks.
  • 08:13Although this may have to be a
  • 08:15tradition that will have to be
  • 08:17re imagined after the pandemic.
  • 08:22Back to some more serious topics.
  • 08:24I'm sure you're also wondering what
  • 08:25benefits we get as residents at Yale.
  • 08:30First off, we have a pretty
  • 08:31competitive salary or paid similarly
  • 08:33to our New York City counterparts,
  • 08:35but don't know the same cost of living.
  • 08:40We have a House staff allowance of
  • 08:42$1400 for all residents and this can
  • 08:45be used for a wide range of things
  • 08:48including conference travel, loops,
  • 08:51textbooks, laptops and cell phones.
  • 08:54We also have an allowance from our section.
  • 08:58Specifically, this ranges from $500
  • 09:00to $1500 depending on your pgy year.
  • 09:04In addition, we also get conference
  • 09:07reimbursement 100% for podium
  • 09:09presentations at major conferences,
  • 09:11or 50% for podium presentations
  • 09:14at other conferences or poster
  • 09:17presentations at any conferences.
  • 09:19We of course also get access to Yale,
  • 09:22including all of its research
  • 09:25and other amenities.
  • 09:26The division also pays for us to
  • 09:30attend both the Intern Bootcamp
  • 09:32an the ASMS Basic course.
  • 09:35We also have access to the Microsurgery Rat
  • 09:39Lab where we can get hands on practice.
  • 09:42Using the femoral vessels of live rats.
  • 09:46We also have access to the Yale Cadaver
  • 09:49Lab where we often have training
  • 09:52through industry sponsored courses.
  • 09:54And finally,
  • 09:55one of the best things we have
  • 09:57access to his doctor,
  • 09:58Thompson's nonprofit organization handheld,
  • 10:00which provides much needed care to
  • 10:03patients who may not otherwise receive it.
  • 10:05At least two or three residents are
  • 10:07able to go a doctor Thompson abroad
  • 10:10every spring and we always try to make
  • 10:13sure everyone who is interested is able
  • 10:15to attend sometime during their time here.
  • 10:17This is a picture of hand
  • 10:20help in 2019 in Peru.
  • 10:22Of note,
  • 10:22we're also trying to create a
  • 10:24craniofacial counterpart as well,
  • 10:25so look forward to that in the future.
  • 10:31Now enough about work, what do we do for fun?
  • 10:36We gotta yell. Sponsored events
  • 10:39such as charity fundraisers.
  • 10:41And happy hours hosted by the
  • 10:43Yale residents and fellow Senate.
  • 10:46We also have what we like to call
  • 10:48mandatory fun like the Annual Boat
  • 10:49Day that we have every summer
  • 10:51during which Doctor Thompson
  • 10:53takes everyone out on his boat.
  • 10:58Even the residents who don't know
  • 11:00how to swim have fun, kind of.
  • 11:05Birthday is always followed by
  • 11:06dinner with the Residents, Fellows,
  • 11:09medical students and staff.
  • 11:12We also sometimes replicate these
  • 11:14days by going out on Doctor
  • 11:16Steinbacher's boat for Journal
  • 11:18clubs while it's still warm.
  • 11:20In addition to both day,
  • 11:22we also have an annual golf day.
  • 11:25Some of us are better at driving
  • 11:27the golf cart then golfing,
  • 11:28but it's always a great time.
  • 11:32This of course is followed
  • 11:35by another dinner together.
  • 11:37Sometimes we skip the activities
  • 11:39and go straight to the food.
  • 11:40This is a lamb roast hosted by Doctor
  • 11:44Cloonan is amazing backyard last year.
  • 11:47We also sometimes get together for trivia
  • 11:49nights paintballing with Doctor Alperovich,
  • 11:52or to listen to our Co
  • 11:55residence in their band.
  • 11:57Now is actually a good time to talk about
  • 12:00what it's like to live in New Haven,
  • 12:01which is actually the first
  • 12:04planned city in America.
  • 12:05New Haven is obviously in New England,
  • 12:07which is a great place to
  • 12:10experience all Four Seasons.
  • 12:12Many of us live downtown,
  • 12:14like in this building that can
  • 12:15be seen from a hospital window,
  • 12:18although those with families or
  • 12:20those who want more space tend
  • 12:22to opt to live slightly further
  • 12:24away in the neighboring towns.
  • 12:26The biggest part of New
  • 12:29Haven is undoubtably Yale.
  • 12:30Through Yale,
  • 12:31we have access to amazing libraries.
  • 12:36As well as theater.
  • 12:39Here is a group of us gathered
  • 12:41after a show at the Yale Cabaret.
  • 12:44We also have great art.
  • 12:47And then there's also the Harvard Yale game,
  • 12:49which is at Yale every other year
  • 12:51and gives us another excuse to get
  • 12:53together and show some spirit.
  • 12:58Outside of yell, there
  • 12:59is also a great hiking.
  • 13:04And sailing such as to the Thimble
  • 13:07Islands in the Long Island Sound.
  • 13:10You can also spend time sitting on the beach.
  • 13:16We're going fishing or even
  • 13:18surfing just a few hours away.
  • 13:21They are also great farms and orchards
  • 13:24for things like Apple picking.
  • 13:26And more arts and music at
  • 13:28things like the International
  • 13:30Festival for Arts and Ideas.
  • 13:34If you want to relive your childhood,
  • 13:36you can even go to Six Flags,
  • 13:37which is a favorite thing for a number
  • 13:39of our residents to do a few years ago.
  • 13:43Most importantly, however,
  • 13:45livability.com actually named
  • 13:47New Haven as the best foodie
  • 13:50city in the country in 2014.
  • 13:52We have no mostly for brick oven pizza,
  • 13:55but you can find much more at the more
  • 13:57than 120 restaurants located within
  • 13:59two blocks of the New Haven Green.
  • 14:15But if you're tired,
  • 14:16New Haven and just want to get out
  • 14:18of town for the weekend, Boston,
  • 14:20New York City and Philadelphia,
  • 14:21or just a few hours away.
  • 14:25So to put it all together,
  • 14:27yellow plastic surgery as a family,
  • 14:29we like to work hard and play hard.
  • 14:33We enjoy spending time
  • 14:34with each other at work.
  • 14:39An out of work.
  • 14:42We're excited to meet our
  • 14:44newest members of the team.
