Community Services Network

Who We Are

The Community Services Network (CSN) is a collaborative of sixteen community-based organizations that provide a broad array of coordinated behavioral health services to people throughout the greater New Haven area. Funded by the Connecticut Department of Mental Health and Addiction Services (DMHAS), the CSN serves more than 5,000 individuals annually, primarily from the towns of New Haven, Hamden, Bethany and Woodbridge.

The CSN is coordinated by the Connecticut Mental Health Center (CMHC), which is Lead Mental Health Authority for the New Haven area. CMHC is a joint endeavor of DMHAS and the Yale University School of Medicine, Department of Psychiatry.

Our Mission

The mission of the CSN is to help individuals and families pursue and enjoy fulfilling lives in the community through the coordination of recovery-oriented behavioral health services.

• Please submit a single copy of this form for referral to one or more services.
• DO NOT LEAVE ANY FIELD BLANK. Put N/A in any field not appropriate. Incomplete referrals cannot be processed and will be returned.
• All referrals require A CURRENT RELEASE OF INFORMATION (ROI) from the referring provider and signed to “Community Service Network (CSN)” by the client. This form will be submitted separately, either uploaded electronically or by fax.
• All referrals require clinical information (more detailed for Residential) which will likely need to be uploaded or faxed separately. Please see Clinical Information Section (page 4) for more details and instructions.
• Residential referrals are for individuals seeking transitional services only and who have a minimum income of $771 monthly to cover rent payment.
• This packet should always be completed in collaboration with your client.
• Clinicians should always maintain a copy of the submitted referral in their client’s file.
• Referrals expire one year from date of submission.
