The Sight Outcomes Research Collaborative (SOURCE) Ophthalmology EHR Repository

SOURCE is

a consortium of academic ophthalmology departments on EPIC EHR who are contributing data on patients with ocular diseases seen at their institutions
  • Academic ophthalmology departments on EPIC HER consist of:
    • Patient Demographics
    • Medications
    • Data from Operative Reports
    • Laboratory Test Results
    • Pathology Results
    • Patient Reported Outcome Data
    • Billing & Administrative Data
    • Structured Data from Clinic Visits (VA, IOP)

SOURCE Data Extraction Work Flow

  • Local Server
    • Local Server Pulls data from Epic
    • All of the data in SOURCE will be completely de-identified through the PHI Removal Software, handled as follows:
      • First name, last name -> Fully omitted
      • Date of birth -> Date shifted
      • Dates of service -> Date shifted
      • Medical record numbers -> Fully omitted
      • Zip codes -> Only first 3 digits of 5 or 9-digit zip codes retained
    • Data is sent to a secure HIPAA compliant server at SOURCE Data Center
    • Data cleaning/aggregation activities occur
    • Finally, creation of derived variables
