The Sight Outcomes Research Collaborative (SOURCE) Ophthalmology EHR Repository
SOURCE is
a consortium of academic ophthalmology departments on EPIC EHR who are contributing data on patients with ocular diseases seen at their institutions
- Academic ophthalmology departments on EPIC HER consist of:
- Patient Demographics
- Medications
- Data from Operative Reports
- Laboratory Test Results
- Pathology Results
- Patient Reported Outcome Data
- Billing & Administrative Data
- Structured Data from Clinic Visits (VA, IOP)
SOURCE Data Extraction Work Flow
- Local Server
- Local Server Pulls data from Epic
- All of the data in SOURCE will be completely de-identified through the PHI Removal Software, handled as follows:
- First name, last name -> Fully omitted
- Date of birth -> Date shifted
- Dates of service -> Date shifted
- Medical record numbers -> Fully omitted
- Zip codes -> Only first 3 digits of 5 or 9-digit zip codes retained
- Data is sent to a secure HIPAA compliant server at SOURCE Data Center
- Data cleaning/aggregation activities occur
- Finally, creation of derived variables
