Terms and Conditions

Yale New Haven Health System, referred to together with its affiliated hospitals and community physicians who utilize Yale New Haven Health System’s electronic medical records system (“YNHHS”), provides you with access to MyChart. MyChart is a secure, web-based service that lets YNHHS patients, or another person chosen by the patient (a “proxy”), have secure web-based access to parts of their YNHHS electronic medical record.

MyChart is a selected part of the information in your electronic medical record, such as certain prescriptions, basic test results, summary medical history, appointments, and similar information. YNHHS or your physician may decide which types of information to release to MyChart.

Security, Confidentiality and Usage Guidelines for MyChart

  • MyChart is designed as a secure internet-based record where you may
    receive confidential medical information about yourself.
  • Once you sign up for MyChart, you will be given a one-time only code
    that you will use to start your account.
  • You will then create your own MyChart ID, PASSWORD and PASSWORD hint.

Password Guidelines

  • Please DO NOT SHARE your password with anyone. The MyChart ID and PASSWORD you choose when you first log-on to MyChart are the way for you to protect the confidentiality of your information.
  • You are responsible for remembering your MyChart ID and PASSWORD and for keeping them confidential.
  • You should write down your MyChart ID and PASSWORD hint and keep them in a safe place.

Activation Guidelines

  • Once you have activated your MyChart account, MyChart will become the main way for you to get important messages from your doctor, like selected test results and other personal medical information. You will still receive communications via the United States Postal Service, e-mail, or other means.
  • You MUST provide us with your email address when activating your MyChart account and keep your email address current in MyChart.
  • You need to make sure your email address is up to date so that you will be notified about new information right away.

Communication Process

  • Check your email on a regular basis for MyChart communications.
  • No confidential information will be sent to your email, but you will receive email notification whenever a new result has been released to your MyChart account.
  • You will receive your personal health information from your doctor in your personal MyChart account.
  • Please never send any messages requiring urgent attention using MyChart.

Urgent Medical Matters

  • For all urgent medical matters, contact your doctor's office, go to an emergency room, or call 911.
  • For all medical emergencies, immediately dial 911

MyChart User Responsibilities

By using MyChart you agree to be bound by all the terms and conditions of this agreement and our Privacy Policy as posted at https://www.ynhh.org/patients-visitors/patient-rights-responsibilities/your-privacy.aspx. Please read this Agreement and Privacy Policy periodically as we may change them at any time. By enrolling in MyChart, you agree that any changes will affect you as soon as they are posted.

MyChart is a communication service offered as a convenience to patients of YNHHS . You understand that it provides you with online access to parts of your YNHHS electronic medical record, and that you are solely responsible for any sharing of MyChart content that you communicate to others. You understand that messages will be routed to the appropriate department as necessary for handling, and therefore YNHHS employees, other than your healthcare provider may be involved in addressing your request. Therefore use of MyChart may not be appropriate if there is sensitive information that you would only wish to discuss directly with your doctor.

You understand that you may receive messages, test results and other communications delivered via MyChart. Therefore, you may not receive duplicate written reports of this information from YNHHS. You understand that not all of your YNHHS medical record may appear on your MyChart. Posting of information is at the discretion of your individual doctor(s).

YNHHS reserves the right in the future to charge a fee for the use of MyChart. You will be notified of this change and asked to re-apply if you agree to the fee and wish to continue with this service.

You can discontinue your use of MyChart at any time by contacting MyChartSupport@ynhh.org.

You agree that you will not communicate any offensive material on this site or otherwise through the MyChart service. If we determine that you have violated this agreement or abused the use of this service, we reserve the right, at our sole discretion, to stop your use of the MyChart service. You will be notified if your MyChart service is discontinued.

Privacy Policy

YNHHS understands the importance of protecting the confidentiality of your medical information. To that end, we have detailed policies and procedures regarding access to all medical records that are designed to safeguard your privacy. These policies explain the circumstances under which your medical information may be released to parties outside the System. Our staff is educated about these policies and procedures and the appropriate use of medical information.

Your Medical Record

As we provide your health care, we must keep records of your medical history, current conditions, treatment plan and all treatments given, including the results of all tests, procedures and therapies. Messages you send via MyChart may be made part of your permanent medical record at the discretion of YNHHS, your doctor, or support staff. Once it is made part of your record, it will be accessible to current and future staff members of YNHHS who are appropriately involved with your care. All electronic medical record content generated by YNHHS is subject to state and federal laws governing the security and confidentiality of medical records.

Messaging

As explained above, MyChart should never be used for urgent matters. Delays can occur based upon volume, availability of staff and the complexity of your condition. FOR ALL MEDICAL EMERGENCIES, IMMEDIATELY DIAL 911.

Terms and Conditions of Use

MyChart may be routed to other authorized caregivers within Yale New Haven Health System and its affiliates in order to facilitate a timely response to your request. All communications between you and your Yale New Haven Health System health care team using MyChart are carried over a secure, encrypted connection directly into the Yale New Haven Health System electronic medical record. While you may receive Internet e-mail messages notifying you of new messages in your MyChart Inbox, these e-mails will not contain any confidential medical information.

Restrictions

MyChart will not include diagnosis or testing information pertaining to Mental Health, Alcohol and Substance Abuse, genetic testing information, and HIV.

Parents and legal guardians may request access to their minor child’s health information through MyChart. This type of access is known as proxy access. A parent’s proxy access for a child will expire when the child turns 18. The amount of information the parent will be able to see in MyChart depends on the age and legal status of the minor. The limits on MyChart access does not affect any other right the parent may have to obtain a minor child’s medical records.

All content included on Yale New Haven Health System Web sites, including, but not limited to, text, photographs, graphics, button icons, images, artwork, names, logos, trademarks, service marks and data (the "Content"), in any form including the compilation thereof, are protected by U.S. and international copyright law and conventions. Except as set forth below, direct or indirect reproduction of the Content, in whole or in part, by any means, is prohibited without the express written consent of Yale New Haven Health System.

Security

This Web site uses state-of-the art encryption software. Once your medical information is received by us, it will be treated as confidential and given the same protection that all other YNHHS medical records are given. However we cannot guarantee that information will be perfectly protected. Information will be transmitted over a medium that is beyond the control of Yale New Haven Health System and its affiliates.

YOU HEREBY EXPRESSLY ASSUME THE SOLE RISK OF ANY UNAUTHORIZED DISCLOSURE OR INTENTIONAL INTRUSION, OR OF ANY DELAY, FAILURE, INTERRUPTION OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH THE USE OF THIS SERVICE.

General

By using MyChart or any YNHHS web site, you agree that the laws of the state of Connecticut, without regard to principles of conflict of laws, will govern these Terms and Conditions of Use and any dispute that might arise.

Urgent Medical Matters

Please do not use MyChart to send any messages requiring urgent attention. For urgent medical matters, contact your doctor's office.
