Transitions Clinic
Transitions Clinic – New Haven is a primary care clinic based out of the New Haven Primary Care Consortium (NHPCC) that cares for individuals recently released from prison and jail who are living with chronic health conditions as well as substance use disorder. Transitions is staffed by a clinician and two community health workers who have a history of incarceration, hosts Collaborative Behavioral Health and Addiction Medicine in Primary Care (CHAMP) trainees, and also incorporates pro bono work with law students through a medical-legal partnership. The clinic is part of the national network, the Transitions Clinic Network and affiliated with the SEICHE Center for Health and Justice.
