In some cases, CDS implementers may need to implement substantial changes to workflow to enable CDSS to operate effectively. At Yale, a key challenge had been low-use of the EHR during clinic, which relegates CDS to a data entry/decision validation tool at the end of the clinical session. To address this, Yale changed workflow to capture patient information directly from patients using iPad technology, to be fed into the EHR automatically, potentially diminishing the clinicians’ interviewing work.
The new application automates the paper-based intake process that limited the potential for using CDS effectively during the patient examination. This represented a significant redesign of the clinical workflow, which was not included in the scope of the initial CDS implementation.
