Diagnosing and Targeting Mechanisms of Diuretic Resistance in Heart Failure

What is the purpose of this trial?
The overarching goal of this study is to create and validate pragmatic tools to rapidly detect and then define the mechanism of diuretic resistance, allowing individualized therapy for acute decompensated heart failure.
The scientific aims of this study are:
- to develop inexpensive and efficient tools to predict diuretic response in a reliable and timely manner,
- to understand the prevalence of therapeutically targetable mechanisms of diuretic resistance using endogenous lithium clearance, a "gold standard" technique to query in vivo proximal tubular/loop of Henle sodium handling,
- to develop methodology to differentiate diuretic resistance mechanisms using common/inexpensive laboratory tests
- to provide proof of concept that mechanistically tailored diuretic therapy can improve natriuresis and
- to establish a biorepository of urine, blood, and DNA specimens for future study of mechanisms and the impact of diuretic resistance.
