- Methods
- The researchers analyzed tumor samples from 195 mesothelioma patients treated between 2010 and 2022. Genetic testing was conducted to identify mutations in 50 cancer-associated genes. Survival data were collected, and statistical analyses were performed to evaluate the impact of specific mutations on overall survival. The cohort included 137 patients with pleural mesothelioma and 58 with peritoneal mesothelioma.

- Key mutations linked to worse survival in pleural mesothelioma included NF2 (found in 24% of patients), TERT (9%), and CDKN2A (26%). Specifically, truncating mutations in the NF2 gene—mutations that result in incomplete, nonfunctional proteins—were associated with significantly higher mortality compared to other NF2 mutations. TERT mutations were also independently linked to shorter survival. In contrast, these mutations did not significantly affect survival in peritoneal mesothelioma patients, suggesting that prognosis in this subtype may be more influenced by treatment factors.

- These findings highlight the importance of genetic testing in pleural mesothelioma to improve prognostication and guide enrollment in clinical trials for targeted therapies. Specifically, identifying NF2 truncating mutations could help select patients for trials targeting the NF2 pathway. For peritoneal mesothelioma, treatment decisions may continue to rely more on clinical factors like tumor burden and resectability, as genetic mutations showed less prognostic impact.

- The authors recommend further studies to validate these findings in larger, multi-institutional cohorts.
