Title: Door-to-Balloon Time for Withdrawal: Evidence & New Approaches for Acute Withdrawal in the Fentanyl Era

1. Describe the clinical and epidemiologic significance of patient-directed discharge and undertreated opioid withdrawal among hospitalized patients with OUD in the fentanyl era.

2. Evaluate the evidence linking early cumulative methadone dose to risk of patient-directed discharge, including the strengths and limitations of observational cohort data.

3. Apply a time-to-treatment ("door-to-balloon") framework to acute withdrawal, identifying system-level strategies to deliver adequate MOUD earlier in the hospital course.
