Form new relationships

  • Reframe the process of meeting new providers and try to be open to their new style;
  • Be patient; Give them a chance to get to know you, and you them.
  • Be optimistic and hopeful that you can find another health care team with caring providers who can help you continue to successfully manage your diabetes

Orient to the future

  • You are becoming your own boss; It is time for you to be planning on how to take more action on your future
  • Set small and realistic goals for what you will do. (For example; Even though I hate seeing my high blood sugars, I will test my blood sugar one more time a day).

Reach out for support

  • Seek out support groups for teens and young adults with diabetes; look in clinics, health centers, or hospitals, and ask your diabetes care provider for help to find one that works for you.
  • Find a pen pal or email buddy. Sometimes it is good to share how you feel about having diabetes with someone else.
  • Tell your current and/or new health care team about how you are feeling and share with them any worries or concerns you have about changing providers.

Gain new knowledge

  • Look through the links on our website to find more information about transitioning to adult health care.
  • Check out the information we have on eating healthy, carbohydrate counting, navigating pumps, managing stress and more.
  • Ask questions!! Assert yourself and ask if something you hear doesn’t make sense.

Embrace life

  • Celebrate your accomplishments. Share with family and friends your successes.
  • Reward yourself when you reach each personal goal that you have set. Don’t wait until they are all met because you do not have to reach all your goals at once. Start with one or two, and build on your success.
  • Do something fun and enjoy your healthy self!
