Provides social work services to Yale Internal Medicine Associates (YIMA) patients and families for physical, social, emotional, and economic problems. The social work function in this setting focuses on the following:
- Completing comprehensive bio-psychosocial assessments of patients, with continual assessment of needs and coping
- Regular collaboration with interdisciplinary team comprised of Medical Doctors, APRNs, MAs, RNs/LPNs, Care Managers, Mental Health Collaborative Care Program's Behavioral Health Manager and consulting Psychiatrist as it relates to development of appropriate patient care plan
- Mobilizing and coordinating the efforts of family, friends, community partners, and the interdisciplinary team on behalf of patients
- Providing clinical case management, including advocating on behalf of patient and/or family in health-related matters and for the procurement of services and/or entitled benefits
- Providing supportive counseling to patient and/or family when appropriate
- Completing safety risk assessments and follow up as needed
