- 00:00Hi, everyone.
- 00:01Welcome to Yale Hospice and Palliative
- 00:04Medicine Fellowship program.
- 00:06I'm doctor Alex Choi,
- 00:07one of our program faculty,
- 00:09and I'll be your guide today.
- 00:11We're excited to meet you,
- 00:12and we hope this video will give you
- 00:14a sense of our fellowship experience.
- 00:16Let's start with a day in the
- 00:18life of a palliative care fellow
- 00:19on our inpatient consult service.
- 00:24Our fellows begin their day in
- 00:25the fellows workroom, which they
- 00:27share with the heme-onc fellows.
- 00:29Here they review charts and
- 00:30prepare for the day. It's important
- 00:32to have a dedicated space to focus.
- 00:36Then we have our daily interdisciplinary
- 00:38morning huddle to run the list
- 00:40and distribute consults. And yes,
- 00:42we're always this happy at work.
- 00:48After the morning Huddle,
- 00:49fellows are welcome to stop by our
- 00:51Palliative Care office where they
- 00:53can grab a bottle of water and
- 00:54hang out with the attendings and
- 00:56APP's before seeing patients. Hi, Christina.
- 01:01Our inpatient consults come
- 01:02from all areas of the hospital.
- 01:04Let's stop here at the fountain on
- 01:05the 1st floor where we can see the
- 01:08four pavilions of the hospital.
- 01:09W Pavilion has the CT,
- 01:11ICU and transplant unit.
- 01:13It also houses Pediatrics.
- 01:15S Pavilion has the ED and the neurology,
- 01:18cardiology and surgical units.
- 01:21E Pavilion is home to the Med surg
- 01:23beds and N pavilion houses the
- 01:26medical ICU and several oncology units
- 01:28including separate floors for hematology.
- 01:30Oncology and GYN-ONC.
- 01:32After seeing patients, fellows round
- 01:34with their attendings where learning
- 01:36is always the priority and of course
- 01:38we want Fellows to feel supported.
- 01:41It's time for lunch.
- 01:43The cafeteria is fine,
- 01:44but you can't miss out on the food
- 01:46carts in front of the medical school.
- 01:48There's halal, arepas,
- 01:50padmi bibimbap, salads and
- 01:53lots more to choose from.
- 01:57In the afternoon, fellows continue bedside
- 01:59rounds unless they are scheduled for their
- 02:02weekly half day clinic session. Here's
- 02:04a look at our palliative care clinic on the
- 02:064th floor of North Pavilion.
- 02:09Fellows are assigned to a different
- 02:10attending for each half of the year.
- 02:19Wow, this guy seems really friendly.
- 02:23And that's it for the day.
- 02:24Wait, is that sunlight?
- 02:28During your time at Yale,
- 02:29you'll appreciate the many unique things
- 02:32that are available at our institution.
- 02:34One example is the oncology
- 02:35extended care clinic or ECC,
- 02:38a dedicated space within the hospital
- 02:40that allows our oncology patients to
- 02:43bypass the ED and receive urgent care.
- 02:46During the height of the COVID-19 pandemic,
- 02:49this space was converted to the
- 02:50supportive care unit where our
- 02:52palliative care team took care of
- 02:54patients who were comfort measures only.
- 02:57On the 7th floor is
- 02:58the Healing Garden, another unique place
- 03:00available to patients, families and staff.
- 03:07Though our fellows spend a large part
- 03:08of the year at Yale New Haven Hospital,
- 03:10they also have a diverse exposure to several
- 03:13Hospice and Palliative care settings.
- 03:15They spend three weeks with the palliative
- 03:17care team at the West Haven VA and
- 03:20four weeks at Connecticut Hospice,
- 03:21the first Hospice in the US.
- 03:24Fellows also rotate through
- 03:25Middlesex Hospice for four weeks,
- 03:27and later in the year they spent two
- 03:30weeks learning about the role of Hospice
- 03:32medical director with Doctor Shaban,
- 03:33a Regional Medical director for VITAS.
- 03:36Fellows also spend 4 weeks with the
- 03:38Sunflower palliative care team at
- 03:40Connecticut Children's Hospital.
- 03:46Outside of work,
- 03:46there are plenty of fun things to do.
- 03:48New Haven is centrally located
- 03:50between Boston and New York City,
- 03:52making it convenient to access
- 03:54either of them on the weekends.
- 03:55But you don't have to go that far.
- 03:58Art museums.
- 03:59Parks, beaches, and of course,
- 04:02the famous Connecticut Apizz.
- 04:05New Haven has a diverse culinary
- 04:07scene with a variety of authentic
- 04:09cuisines from around the world.
- 04:21The Yale University Campus is also
- 04:23located a few blocks from the hospital.
- 04:26New Haven is fun to explore at night
- 04:28too, with options for restaurants,
- 04:30bars, music and theater venues.
- 04:33We thank the various organizations
- 04:35who are dedicated to enriching
- 04:37the culture of downtown New Haven.
- 04:38For those who are looking
- 04:39to get out of the city,
- 04:41the Connecticut shoreline is home to
- 04:43beautiful views and tasty lobster rolls.
- 04:45Families also do day trips to
- 04:46the beautiful little towns in the
- 04:48northwest corner of the state.
