Project mCHARTS
The mCHARTS (Mobile Co-location of HIV-Related Activities with Resources and Transitional Services) initiative represents an innovative model of integrated care that will deploy evidence-based practices and adapt them to enhance existing services, primarily for racial/ethnic minorities and people at risk for or living with HIV (PARLWH) who are homeless or with unstable housing, and create a collocated "Patient-Centered Medical Home (PCMH) in New Haven, CT. The mCHARTS program includes the co-location of services between a mobile medical clinic (MMC) operated by the Yale AIDS Program (YAP), which provides HIV and HCV treatment and prevention services combined with integrated substance abuse and mental health treatment with Liberty Community Services (LCS), the city's largest subsidized housing and HIV prevention and testing program for PARLWH.
