Community Health Care Van

The Community Health Care Van (CHCV) is both a storefront clinic located at 270 Congress Ave and a mobile medical clinic that travels around New Haven. CHCV was started by Frederick Altice, MD in 1993.

Healthcare

Psychiatric Care (provided at 270 Congress Ave)

  • Psychiatric assessments
  • Buprenorphine and other medication-assisted treatment (MAT) for substance use disorders
  • Brief psychotherapy
  • Referrals to more intensive treatment as needed
  • Walk-ins welcome on Mondays, Wednesday mornings, and Fridays, or call for an appointment (203) 764-9995.

Physical Care (provided at 270 Congress Ave and on the CHCV Mobile Clinic)

  • Infectious disease testing and treatment
  • PrEP and PEP prescriptions
  • Wound care

Pediatric Care

  • Infant weight checks and physical exams
  • Pediatric health care advice and developmental guidance
  • Bridge to future care for mom and baby

Syringe Services (SSP)

At the storefront (270 Congress Ave):

At our harm reduction vending machines:

  1. Syringes, Narcan, and other supplies can be obtained for free from our harm reduction vending machines.
  2. We have two vending machines located at:
  • APNH 1302 Chapel St, New Haven, CT 06511
    • Open 24/7
  • The APT Foundation in West Haven, 184 Front Ave, West Haven, CT 06516
    • Open when APT is open: M-Sat 5:30am-1pm. Closed Sundays

Social Services

Assistance in referrals to:

  1. methadone clinics
  2. inpatient detox
  3. HIV re-entry case management
  4. other rehabilitation programs

Drug Checking

  • We analyze the contents of donated drug samples to provide feedback on the quickly changing drug supply
