Cardio-Rheumatology Program

Yale Preventive Cardiovascular Medicine has partnered with Yale Rheumatology to offer personalized cardiovascular risk assessment and treatment of patients with rheumatologic diseases. The Cardio-Rheumatology program at Yale is the only program in the state of Connecticut dedicated to preventing and managing heart disease in patients who have underlying autoimmune and rheumatologic disorders.

Under the leadership of cardiologist Margaret L. Furman, MD, MPH, in close collaboration with Vaidehi Chowdhary, MBBS, MD, DM, clinical chief of rheumatology, Yale cardiologists and rheumatologists have united to offer patients an innovative multi-disciplinary program aimed at improving outcomes.

The program specializes in the early detection and treatment of heart disease and cardio specific disorders in patients with inflammatory and autoimmune joint disease with a focus on clinical care and patient education. The program treats a highly diverse patient population.

Pressure Points: A Novel Program to Control Blood Pressure

This program offers a simple, personalized approach to managing blood pressure. Patients with uncontrolled blood pressure can enroll in the program, monitor their blood pressure at home, and meet with our team of cardiologists, ObGyn, pharmacists, and nurses - either virtually or in-person - to make changes to the blood pressure medication and support lifestyle goals.

Who is eligible to join:

  • Patients with uncontrolled blood pressure (>140/90 mmHg)
  • Postpartum women with pre-eclampsia, eclampsia or hypertension.

What’s involved:

  • Consultation with a cardiologist and a follow-up visit in 3 months, or sooner if indicated. In between, patients meet with either a nurse or pharmacist to manage the blood pressure using medication and/or lifestyle interventions.
