Individualized Training Pathways

The Yale Critical Care Medicine Fellowship is committed to preparing fellows for successful careers in critical care medicine while recognizing that trainees enter the program with diverse specialty backgrounds and long-term career goals. All fellows complete a comprehensive core curriculum that provides the breadth of knowledge, procedural experience, and multidisciplinary clinical training required for independent practice. Within this framework, we work closely with fellows to individualize elective experiences and, when appropriate, tailor clinical rotations to support specific career goals while maintaining a strong foundation in comprehensive critical care medicine.

Yale offers a formal three-year Nephrology/Critical Care Medicine Track for physicians seeking dual training in both specialties. The program combines two years of nephrology fellowship with one year of critical care medicine fellowship, preparing graduates for board eligibility in both disciplines.

For fellows who are not enrolled in the combined Nephrology/Critical Care Medicine Track, the CCM fellowship can often incorporate nephrology-focused electives that provide additional experience in areas such as continuous kidney replacement therapy, glomerular disease in the ICU, electrolyte and acid-base disorders, and the care of critically ill patients with kidney disease.

For fellows interested in careers that include cardiac critical care, Yale offers opportunities to expand training in collaboration with our Cardiac Intensive Care Unit (CICU) leadership and cardiac critical care faculty, who are members of the Critical Care Medicine Core Faculty.

Individualized training plans may include additional time in the Cardiac Intensive Care Unit (CICU), providing greater experience in the management of acute myocardial infarction, cardiogenic shock, advanced heart failure, mechanical circulatory support, and heart transplantation. Fellows may also expand their experience in the Cardiothoracic Intensive Care Unit (CTICU), with additional exposure to the postoperative management of complex cardiac surgical patients, extracorporeal life support (ECLS/ECMO), and advanced cardiopulmonary physiology.

Elective opportunities can also be tailored to develop advanced echocardiography skills, including both transthoracic (TTE) and transesophageal (TEE) echocardiography, based on each fellow's educational goals.

For physicians entering from Emergency Medicine, Yale offers a two-year Emergency Medicine–Critical Care Medicine (EM-CCM) pathway designed to provide comprehensive multidisciplinary critical care training. The curriculum combines the clinical experiences required for board eligibility with approximately six months of mentored elective and academic time, allowing fellows to pursue individualized interests in clinical care, medical education, research, quality improvement, or professional development.

Training plans are developed collaboratively with fellowship leadership to ensure completion of all required clinical experiences while supporting each fellow's career goals. Graduates who meet ABEM requirements are eligible to obtain subspecialty certification in Critical Care Medicine through the American Board of Emergency Medicine.

Individualized training extends beyond these formal pathways. Fellows pursuing careers that combine critical care with other Internal Medicine subspecialties may be able to incorporate electives that complement their anticipated scope of practice.

Examples include additional experience in advanced liver disease or transplant hepatology for gastroenterology-trained fellows, expanded exposure to the care of immunocompromised patients for infectious diseases physicians, or other specialty-specific electives developed in collaboration with faculty mentors. Educational plans are individualized on a case-by-case basis while preserving the comprehensive clinical experiences that form the foundation of the Yale Critical Care Medicine Fellowship.
