Connecticut Older Adult Collaboration for Health (COACH) Planning for Appropriate Transitions from Home (PATH) PATH (Pathways for Appropriate Transitions from Home) is a guide for long term care planning. It gives caregivers of people with dementia the information they need to decide whether the person they care for should stay at home or move to a senior living community.
