Other Clinical Partners

The Yale Program in Addiction Medicine partners with several other clinical services housed within the university or with community-based organizations.

Cornell Scott Hill Health Street Medicine Program

The CSHHC Street Medicine Program provides direct medical outreach to individuals experiencing homelessness, on the street, in shelters and encampments, and service sites (e.g., soup kitchens, drop-in centers). Staffed by physicians and advanced practice providers, the street medicine team provides both medical and behavioral health care, as well as linkage to wraparound including housing assistance. CHAMP residents and PA students are among trainees hosted by the program for clinical rotations.

The New Haven Primary Care Consortium (NHPCC)

The NHPCC offers primary care for children and adults. In collaboration with Yale New Haven Health and Fair Haven Community Health Care, Cornell Scott Hill Health Center (150 Sargent Drive, New Haven, CT 06510) oversees the adult medicine program that offers primary care, women’s care, and addiction treatment services for adults ages 19 and over. The Addiction Recovery Clinic and Transitions Clinic are embedded within the NHPCC and offer treatment services, including buprenorphine treatment and brief counseling. Several clinicians affiliated with the Yale Program in Addiction Medicine provide addiction treatment in primary care, or clinical services through the ARC or Transitions, at the NHPCC
