Survivorship Clinic

New treatment options and innovative cancer therapies are helping cancer patients to live healthy lives long after their cancer has been cured. The growing number of cancer survivors requires new resources and healthcare specialists to care for the special concerns cancer survivors may have, including medical, social, and emotional worries. The Survivorship Clinic is the first dedicated, multidisciplinary resource for adult cancer survivors in the State of Connecticut to provide patients and their families with vital information on cancer prevention, wellness, supportive services, and the latest health research related to cancer survivorship.

Specialized Team Care

The Survivorship Clinic is comprised of a team of Yale Cancer Center’s leading clinicians and scientists. The primary focus is on providing guidance and direction to empower survivors to take steps to maximize their health, quality of life, and longevity. Additionally, the team educates survivors on the prevention, detection, and treatment of complications resulting from cancer treatment. At the Survivorship Clinic, an oncologist, a nutritionist, an exercise therapist, and a social worker monitor patients. A survivorship program coordinator supports cancer survivors throughout their visit and follow-up consultations.

Customized Treatment Plans

Prior to the clinic visit, the team reviews the patient’s past cancer diagnosis and treatment history, including cumulative doses of chemotherapy and radiation therapy. This information helps to design an individualized treatment summary and survivorship care plan for each patient. On the day of the clinic visit, patients undergo a focused evaluation with customized recommendations in the following areas:

  • Individualized surveillance and screening guidelines and medical provider identification
  • Wellness education tailored to cancer type and treatment history
  • Psychosocial support for survivors and their families
  • Nutritional counseling
  • Exercise and fitness recommendations

At the conclusion of the visit, patients leave the Survivorship Clinic with a written plan tailored to their specific needs and concerns. A copy of this plan is also sent to their referring physician or primary caregiver.

Support Services

Patients at the Survivorship Clinic also have the opportunity to receive referral-based services for cancer survivors. These services include:

  • Genetic counseling
  • Community mental health providers
  • Complementary therapies including reiki, massage, and guided imagery
  • Neurocognitive assessment
  • Sexual health
  • Tobacco Treatment
  • Support groups of all varieties
