Registration Forms
The GE Centricity Business Course Registration/Security Form MUST be completed for initial access to the GE CB system.
The courses for initial access include Patient Inquiry, Scheduling Add/Edit Web or Registration Add/Edit Web. Users must have a valid Yale NETID or must request an affiliate NETID through their business office.

Scheduling Location Forms
To access a providers schedule to make appointments, view schedules or print scheduling reports, you must complete a locations form for your department.
Please check off the appropriate locations and associated departments. If this is a new user please indicate if the user should have access to Quick Registration, Overbook or if they are Reporting/View Only.
