A Model Program for Community Health Centers

This Donaghue Foundation funded project emerged as an outgrowth of discussions with community health practitioners and is a collaboration between the Yale Diabetes Program and the Fair Haven Community Health Center (FHCHC). Clinicians at FHCHC have identified over 2,000 minority women under age 55 with risk factors for T2DM. This project will determine whether a transdisciplinary team of Yale clinical investigators and primary healthcare providers at a community health center can effectively implement an intensive lifestyle intervention to improve the health of patients at high risk for T2DM and other clinical complications of obesity, as well as their children.
