Funeral Arrangements

The family must make arrangements with a licensed funeral director within three days of the patient passing. The Department of Consumer Protection has information on funeral homes. Funeral directors are the best people to contact regarding financial concerns and potential benefits. Public Act 04-255 specifies what is expected of funeral directors in the state of Connecticut. Yale-New Haven Hospital cannot recommend a funeral home or cremation service.

The Autopsy Service is responsible for ensuring that the hospital portion of the death certificate is filled out correctly. The funeral director will complete the death certificate and file it with Vital Statistics. The funeral director provides the family with one certified copy of the death certificate. Please visit the Department of Public Health website for questions regarding how to obtain additional copies of the death certificate. The Autopsy Service cannot release the death certificate to the family directly.
