Prepare for your Appointment

Prior to your appointment in our clinics, we ask that you please either mail or bring the following materials if they are available:

• Any results of previous scans such as CAT scans or MRI scans. If possible, we would prefer to have the actual images either in hard-copy format or on a CD-ROM.
• Any results of previous nerve conduction studies, electromyography studies or electroencephalography studies.
• Any notes or information regarding previous brain surgeries or back surgeries.
• Any notes from other physicians regarding the condition for which we will be evaluating you.
