Biography

Board certified in Internal Medicine, Dr. Fogerty joined the Academic Hospitalist team in 2011. His scholarly work has a strong emphasis in educating medical professionals in safety and quality issues, with a focus on cost-awareness. Since mid-2013, Dr. Fogerty has served as Co-Chief of the Generalist Firm, along with Dr. Patrick O’Connor, Chief of General Medicine.

Most recently in August 2014, he co-author a paper which illuminated the need for better follow-through with patients after they leave the hospital. The JAMA IM study found that holes in continuity of patient care due lack of communication with their doctors often resulted in readmission, especially in cases such of acute coronary syndrome, heart failure, or pneumonia. In his role as a Yale Hospitalist, Dr. Fogerty strives to not only uncover the issues that present barriers to better outcomes for patients, but to find solutions as a result of this awareness.

Dr. Fogerty is also co-chair of the Yale-New Haven Hospital GME Quality Improvement Committee, and in the spring of 2015 helped lead the launch of a new quality and safety curriculum for all residents and fellows at Yale-New Haven Hospital. He coordinates monthly Morbidity & Mortality sessions for the Department of Medicine, is the course director for the Internal Medicine Residency Quality and Safety elective rotation, and coordinates the medical student Morbidity & Mortality session for the Internal Medicine clerkship. He is active in both the Society of Hospital Medicine and the Society of General Internal Medicine, holding regional and national leadership positions. In 2013, he won the American Board of Internal Medicine Foundation/Costs of Care Teaching Value and Choosing Wisely competition for his work in developing a novel cost-awareness education tool that has been implemented nationally in Internal Medicine Residency programs.
