Sandy S. Chang MD, MHS

Biographical Info

Dr. Chang’s long-term career goal is to determine how to enhance the clinical management of multimorbid older adults and to deliver processes of care more effectively in order to improve overall health outcomes in this vulnerable population.

She has successfully obtained funding as a research scholar from the John A. Hartford Center of Excellence in Geriatric Medicine at Yale, the Yale Program on Aging and Claude D. Pepper Older Americans Independence Center, and the Yale Center for Clinical Investigation. She has published peer-reviewed articles which have focused on the epidemiology of comorbid chronic inflammatory conditions and frailty; inflammatory biomarkers associated with impaired pulmonary function; and outcomes of multiple co-existing chronic conditions, including chronic obstructive pulmonary disease (COPD) and cognitive impairment, in older adults.

Dr. Chang graduated from the Program in Liberal Medical Education at Brown Medical School in Providence, RI and completed her residency training in family medicine at Christiana Care Health System, which is affiliated with Jefferson Medical College in Philadelphia, PA. She further pursued fellowship training in geriatrics and gerontology at Johns Hopkins University in Baltimore, MD, where she earned a Master of Health Science degree in the Epidemiology of Aging. After completing her fellowship, she joined the Yale medical faculty in the Section of Geriatrics. She also serves as an attending physician in the Acute Care for the Elderly Unit at YNHH and enjoys teaching geriatric medicine and the patient-centered approach to medical students and trainees.
