Visiting Student Elective Program
Please enclose all required documents in a single envelope and mail to:

We will not process incomplete applications.
To apply, please assemble all of the following documents into a single envelope. If your application is not complete and does not included all these documents, we cannot process it.
- Completed application form for the Visiting Student Elective Program.
- Supplemental Form for International Medical School Students, only if you are student from outside the US.
- Letter of support from the dean of your medical school, including verification that you are in your final year of medical school. The dean’s letter should be written on the medical school’s stationary, with the school’s seal and the dean’s original signature. It must be in English or accompanied by an English translation.
- Copy of your current transcript written in English or accompanied by an English translation.
- Curriculum vitae (CV) that lists your education, publications, and other professional accomplishments.
- Letter of recommendation from a member of the faculty at your medical school who has observed your clinical skills. This letter, in English or with English translation, must be submitted with the faculty member’s original signature on your medical school’s official stationary.
- Yale University Pre-entrance Health Form with translation in English, of your immunization record and documentation. lf your PPD is positive, you must INCLUDE a copy of your chest x-ray report in English or with English translation. We require documentation of a quantitative hepatitis B surface antibody titer. A non-quantitative result (i.e. “Positive”) is not acceptable.
- * Proof of health insurance while on the clinical elective at Yale. This certificate should be obtained from your health insurance company.
- Personal statement describing your career goals, how this experience will help you achieve them, and what you have accomplished thus far in pursuit of those goals. Also include what cultural opportunities you will pursue during your stay.
- A record of your TOEFL scores or University of Cambridge ESOL scores if you are a non-native English speaker.
