Transcript Requests & Enrollment and Degree Verifications

The registrar can provide with enrollment and degree verification for current Yale School of Medicine students and graduates with the Doctor of Medicine degree ONLY.  You must complete the Medical Student/MD Graduate Request Form in order to request a transcript or an enrollment or degree verification.  Call or write to the registrar at least five business days before you need the document sent.
