Transcript Requests & Enrollment and Degree Verifications

The registrar will provide students with enrollment and degree verification. All students must complete the Medical Student/MD Graduate Request Form in order to request a transcript or an enrollment or degree verification.  Call or write to the registrar at least five business days before you need the document sent.
