This section lists the forms you will need to submit and provides you with electronic copies that you can download and print. We’ve also included the important deadlines you should keep in mind as well as other advice to make this process as simple as possible.

NOTE: You should print and complete the forms you need from the list below before you send them to the Financial Aid Office. You can send them by mail, fax or as an attachment to an email.

You make decline enrollment in YH Hospitalization / Specialty Care coverage and/or YH Prescription Plus coverage by submitting a waiver online.

  • A wavier must be submitted annually and received by: September 15 for the full year or fall term January 31 for spring term.
  • Waivers will be processed and credits will appear accordingly on your SFAS account within 5 business days. After that time, you may confirm receipt by logging into the YH waiver web application.
