If you have not published your findings from analysis of the Medtronic rhBMP-2 data, the DUA expires one year from the date of approval. The DUA may be subsequently renewed additional times for one-year periods. At the end of the DUA period, please submit a Certificate of Data Disposition within 30 days of the data expiration date. The certificate must be signed by the Principal Investigator. You may scan the signed form and submit it electronically to firstname.lastname@example.org or send it via postal mail to:

If you do publish your findings from analysis of the Medtronic rhBMP-2 data, you may apply to the YODA Project for a 5 year DUA, which will allow you to archive the data for 5 years. The sole purpose of this 5-year data retention DUA is to allow you to address questions about your findings. Additional analyses may not be performed without submitting a new application which includes revised aims. At the end of 5 years, please submit a Certificate of Data Disposition as described above.
